Payment of $45 (CASH OR MONEY ORDER) will be collected at the door. CHECKS AND CREDIT CARDS ARE NOT ACCEPTED AT THE DOOR.

  • No Certificates will be mailed without a case number.
  • Refunds are subject to a $15 cancellation fee.
  • Duplicate certificates are $10.
